You may have access to Blue Shield of California's in-home care program. This program offers convenient, comprehensive home-based care for eligible Blue Shield members in certain counties in California. You may qualify for the program if you are aged 18 or older, managing multiple chronic illnesses, or need extra support to manage your health. If you are eligible and qualify for the in-home care program, you have access to the following services at no additional cost:
- Medical, behavioral, and social services
- 24/7 access to medical professionals, virtually or in person
- In-home urgent care
Once you are enrolled, a home-based care provider will work with your current primary care physician (PCP) and other providers to coordinate your care. This program is available from the following providers:
This is a program for certain eligible Blue Shield of California members. Eligible members will receive a letter inviting them to participate. Eligibility is determined based on health status and needs.
For Blue Shield Commercial plans:
ConcertoCare is independent of Blue Shield of California and is contracted by Blue Shield to provide in-home and virtual care services. These services are not covered benefits of Blue Shield health plans and none of the terms or conditions of Blue Shield health plans apply. Members who are not satisfied with products or services received from ConcertoCare may use Blue Shield's grievance process described in the Grievance Process section of the Evidence of Coverage or Certificate of Insurance/Policy. Blue Shield reserves the right to terminate this program at any time without notice.
Altais is independent of Blue Shield of California. Altais is contracted by Blue Shield to provide program administration services. My Health Medical Group (doing business as BT Health) is managed by Altais to provide in-home and virtual care services. These services are not covered benefits of Blue Shield health plans. None of the terms or conditions of Blue Shield health plans apply. Members who are not satisfied with products or services received from Altais/BT Health may use Blue Shield's grievance process. Members can find the grievance process in the Evidence of Coverage or Certificate of Insurance/Policy. Blue Shield reserves the right to end this program at any time without notice.
For Blue Shield Medicare plans:
The products and services described are neither offered nor guaranteed under Blue Shield of California’s contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding these products and services may be subject to the Blue Shield grievance process. If you encounter a problem, call Customer Service at the number on the back of your Blue Shield ID card.
