Community Health Workers (CHW) are non-clinical staff who can offer support in a variety of ways. CHWs have firsthand experience overcoming various life challenges. Working with someone who has gone through similar experiences can help you overcome your own challenges.

How to request support from a CHW
Contact your primary care provider to get a referral for CHW services.
Covered services
The following CHW services are covered by Blue Shield of California Promise Health Plan:
- Up to 6 hours of CHW services per year
- Up to 2 hours of CHW services per day
The following asthma preventive services (APS) services are covered by Blue Shield of California Promise Health Plan:
- Up to 2 hours of APS daily, two visits per year
- Up to 2 visits per year for in-home environmental trigger assessments
If more CHW or APS services are needed, your doctor or mental health provider must submit a request. Ask your provider to review the Blue Shield Promise provider manual for more information.
