How to request support from a CHW
Contact your primary care provider to get a referral for CHW services.
Covered services
The following CHW services are covered by Blue Shield of California Promise Health Plan:
- Up to 6 hours of CHW services per year
- Up to 2 hours of CHW services per day
The following asthma preventive services (APS) services are covered by Blue Shield of California Promise Health Plan:
- Up to 2 hours of APS daily, two visits per year
- Up to 2 visits per year for in-home environmental trigger assessments
If more CHW or APS services are needed, your doctor or mental health provider must submit a request. Ask your provider to review the Blue Shield Promise provider manual for more information.
